High School Job Shadowing Procedure
- Job shadow participant contacts high school representative
- High school representative and participant review healthcare career job titles together to determine areas of interest
- High school representative reviews the LSHCA High School Job Shadowing Information and Confidentiality Agreement and explains the job shadowing process with the participant. (click here to download form)
Importance of the Confidentiality Agreement:
- Patient Protection
- Respect for the privacy of others
- Breech of confidentiality and the lack of concern for others may involve the participant in legal proceedings
- Participant delivers the following completed documents to the school’s job shadow representative:
- Confidentiality Agreement / Job Shadow form
- Immunization record
(These forms are required before a job shadowing appointment will be scheduled.)
- School representative reviews the above documents. When documents are completed as required, school representative signs Confidentiality Agreement / Job Shadow form, & contacts facility to set up a job shadow for the participant.
- School representative forwards the participant’s documents to the job shadow facility.
- School representative informs participant of the date and location of job shadow and confirms appointment.
- Abiding by the job shadow dress code, participant arrives 10 minutes prior to appointment at facility with photo ID to perform job shadow.
- Upon completion of job shadow, participant completes the Job Shadowing Experience Evaluation Form & fills out ticket to sign up for an annual prize drawing. Both the evaluation form and ticket are handed into the facility representative before leaving. Participant receives a complimentary gift when evaluation is turned in.
Note: If a participant would like to job shadow at more than one facility, an application for each facility will need to be competed. However, the immunization record may be copied and attached to each application.
